Diamond Terminal Commendation Instructions

In order for the facility/site to be recognized at NAPA’s Annual Meeting, nominations must be completed by no later than November 2. 

Participation and renewal
Participation is voluntary and is on an annual basis. Each facility/site may be submitted for the Commendation  every year, at the company’s option.

Possible disqualification
Some questions on the self-assessment deal with aspects of facility/site operations that are felt to be crucial. A “No” answer to any such question is treated as automatic disqualification.
 

Get Started 
To apply for the Diamond Terminal Commendation, you must:

1.    
If you are processing a renewal application, the web page will display a message stating, “Looking up facility history”. Once the database search is complete, click on “View History/Renewal Status.” A pop-up box will appear listing your facilities and the type of renewal that is due. Click on “Renew” next to the facility name to begin the Diamond process.

 For a first-time application for a facility, click on Create New Application.

2.       You will be asked if you understand self-audit liability. If you do, check the box next to “I understand self-audit liability.” If you do not, please contact the Diamond Program Coordinator, toll-free at 888.468.6499.  

3.      Check the box next to I have read and understand the instructions.” 

4.      You will be asked if your facility fits the definition of a terminal as it applies to this application. If it does, check the box.

 

For each application you create, you must complete each screen shown on the Application Checklist. Note that you must first complete the screen called “Facility/Site Information.”  All fields are required.     

Do you qualify?
Complete the forms
:You must fill out several forms that will ask you a series of questions which will then automatically calculate the facility’s results. You may view your score at anytime by clicking on the “Start/Edit Form” button beside each form. To earn the Commendation, you must meet the threshold num­ber for each of the six sections.

      
Upload photographs: This section allows you to upload images. A minimum of 5 images is required for a new or full renewal application. A minimum of 3 images is required for a streamlined renewal. The maximum number you may submit is 10. Photos taken with a digital camera should be at least 960 x 1280 pixels. Click on the desired “Upload Image” button.Then click on the “Browse” button and locate the image you wish to upload. To 
delete an uploaded image, upload another image and the system will automatically replace it.

 

Photograph Requirements: 

Full applications: Photos taken with a digital camera should be at least 960 x 1280 pixels.  Scanned photos are not acceptable. Provide five to ten high-resolution photographs showing different views of the facility and site. One must be a “long shot,” showing the facility/site and its surroundings, one must be an aerial photo, one must display the loading racks, one must show the receiving/distribution areas, and one must be of the facility entrance. In lieu of an aerial photo, you may submit an image from Google Earth provided that it represents the current configuration of the facility. Photographs must be taken during the current calendar year, and/or must represent the current configuration. Photographs may be used for marketing purposes. 

Streamlined renewal applications: Photos taken with a digital camera should be at least 960 x 1280 pixels. Scanned photos are not acceptable. Include one photo of the facility entrance, one long shot photo, and one photo of the receiving/distribution area during operation. Photographs must be taken during the current calendar year. 

All photographs submitted in the Diamond process become the property of NAPA.

Plaque information: If you qualify,
you will receive a handsome plaque (first-time applicants) and materials to assist in publicizing your achievement within your community. The text that you submit here will appear on your plaque. You may order additional plaques if you wish at a cost of $250 each.
 

Affidavits: The system will allow you to download the affidavit(s) required for your application. (Some applications require multiple affidavits.) You must print the affidavit(s), gather the signatures requested, scan the completed documents, and upload them. Affidavits may be signed by the following individuals: an elected official (mayor, councilman, representative, etc.); officer of a nearby homeowner's association; official of a local civic group (Rotary, Kiwanis, Chamber of Commerce); Boy Scout or Girl Scout leader whose troop has toured the facility; a teacher at a nearby school whose class has toured the facility, an engineer from the city or county; SAPA Director; or an employee from the DOT, local highway department, or public works agency. Pay/Submit Application:

-       Once you complete the entire checklist, the “Pay/Submit Application” button will become enabled and you will be able to submit your application and pay online. You may pay online with a credit card or choose "Mail a Check" and mail a check to the NAPA office.
